Locums-to-Perm Nurse Practitioner or Physician Assistant — Hybrid Geriatric Primary Care (Bronx/Brooklyn/Ridgewood, NY)

We’re looking for an experienced Nurse Practitioner or Physician Assistant to join a value-based primary care team serving older adults in New York. This is a locum-to-perm opportunity (conversion expected within 3–6 months) with a hybrid schedule that combines outpatient clinic shifts and home-based visits.

Compensation

• $75–$95 per hour

Schedule & Workload

• Full-time, Monday–Friday (hours guaranteed)
• Low daily patient census to allow focused, high-quality care
• Home visits: typically 3–5 patients per day
• Clinic visits: typically 7–10 patients per day
• Locations: Bronx, Brooklyn, and Ridgewood, NY

What you’ll do

• Provide comprehensive primary care to geriatric patients, including preventive care, chronic disease management, and acute problem treatment
• Assess, diagnose, prescribe, and create individualized care plans
• Order and interpret labs and imaging
• Document thoroughly in the EHR and manage follow-ups and referrals
• Educate patients and caregivers about health, medications, and wellness goals
• Work closely with physicians, nurses, and support staff to coordinate care
• Maintain compliance with all relevant state, federal, and professional regulations

What we’re looking for

• Active NP or PA license in New York
• National board certification
• At least 2 years of primary care experience (geriatric experience preferred)
• Medicaid and Medicare provider IDs required
• Spanish-speaking clinicians encouraged to apply
• Expedited credentialing available
