Floating Family Medicine NP/PA — $100K Base, Multi-Clinic Travel Role in Laurens, SC

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our Family Medicine team as a floating provider, supporting outpatient clinics across Abbeville, Greenwood, Laurens, McCormick, and Saluda counties in South Carolina. This role is ideal for a compassionate, adaptable clinician committed to patient-centered care, prevention, and chronic disease management.

Primary responsibilities

• Deliver comprehensive primary care to patients across the lifespan with an emphasis on wellness, prevention, and evidence-based chronic disease management.
• Travel regularly to assigned family medicine sites within Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care and ensure smooth transitions.
• Provide patient and family education on diagnoses, treatment plans, medications, and self-care.
• Accurately document patient encounters and maintain timely electronic health record entries.

Qualifications

• Graduate of an accredited Nurse Practitioner (APRN/NP) or Physician Assistant program.
• Minimum of 2 years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina license (or eligibility to obtain).
• Certification or eligibility for certification in your specialty (FNP, PA-C).
