Job Description

Join our outpatient clinic in Brooklyn as a full-time clinician delivering compassionate, person-centered care to predominantly Chinese-speaking older adults. You’ll support patients’ functional, clinical, and psychosocial needs with a focus on independence, wellness, and continuity of care — while working within a collaborative, multidisciplinary team.

What you’ll do

  • Provide comprehensive primary care and preventive services to older adults, prioritizing continuity and proactive management.
  • Coordinate care across disciplines, helping develop and implement person-centered goals.
  • Monitor and report changes in patients’ conditions and facilitate appropriate follow-up.
  • Maintain small patient panels to allow time for personalized visits (clinic and field work as needed).

Compensation & benefits

  • Base salary: $155,000 – $175,000 plus a 20% performance incentive
  • Full benefits package: health insurance, retirement plans, CME allowance, and paid time off

Schedule & practice highlights

  • Full-time, Monday–Friday — no nights or weekends
  • Small daily caseload for tailored care (approximately 4–5 patients/day in the field; 7–10/day in the clinic)
  • Collaborative team environment focused on geriatric primary care and proactive management

Who we’re looking for

  • Fluent in Cantonese (Chinese) to effectively serve our patient population
  • Licensed Nurse Practitioner or Physician Assistant in New York
  • Board-certified: ANCC or AANP for NPs; NCCPA for PAs (or equivalent)
  • Minimum of 2 years’ primary care experience
  • Experience or strong interest in geriatrics preferred
  • Strong clinical judgment, excellent communication skills, and solid organizational abilities
