Outpatient Family Medicine Physician — Peoria, AZ

Qualifications:

• MD or DO from an accredited medical school
• Completion of ACGME-accredited residency in Family or Internal Medicine
• Board certified or board eligible in Family or Internal Medicine
• Active, unrestricted Arizona medical license (or eligibility to obtain)
• Current DEA registration for controlled substance prescribing
• Prior family medicine experience preferred
