Locums-to-Perm NP/PA — Geriatric Primary Care (Hybrid Clinic + Home Visits) Brooklyn, $75–$95/hr

Join a value-based primary care practice in Brooklyn focused on geriatric patients. This locum-to-perm opportunity (conversion expected within 3–6 months) is a hybrid role combining outpatient clinic shifts with home-based visits.

Compensation: $75–$95 per hour

Work Schedule and Volume

• Full-time, Monday–Friday (guaranteed hours)
• Low daily patient census
  • Home visits: 3–5 patients/day
  • Clinic: 7–10 patients/day

Key Responsibilities

• Provide comprehensive primary care for older adults, including preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe, and create individualized care plans
• Order and interpret labs and imaging; coordinate specialist referrals and follow-up
• Accurately document care in the EHR and maintain timely, compliant medical records
• Deliver patient education and promote wellness initiatives
• Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care
• Adhere to all applicable state, federal, and professional regulations

Requirements

• Active NP or PA license in New York
• Minimum 2 years’ primary care experience (geriatric experience preferred)
• National board certification; ability to complete expedited credentialing
• Medicaid and Medicare provider ID required
