Join a 16-week contract in Barre, VT (start 06/22/2026) providing hospice and palliative home care on day shift. This RN role requires home health and OASIS proficiency — candidates must be experienced as home care travelers capable of case management. Minimum one year of relevant nursing experience.
What you’ll do:
- Deliver compassionate end-of-life and palliative nursing care, addressing patients’ physical, emotional, and spiritual needs.
- Provide expert pain and symptom management and coordinate comfort-focused interventions.
- Serve as a case manager when needed, completing OASIS documentation and care planning accurately and timely.
- Collaborate with an interdisciplinary team, local physicians, and community providers to optimize patient quality of life.
