Core responsibilities:
- Provide direct patient care in a skilled nursing setting per facility policies
- Administer medications, treatments, and assessments as ordered
- Monitor and document patient condition and report changes to RN/MD
- Assist with ADLs and coordinate patient care tasks with team members
- Maintain accurate medical records and complete required documentation
- Follow infection control and safety protocols
Minimum requirements:
- Active LPN/LVN license for California
- BLS (Basic Life Support) certification
- Minimum 1 year of LPN/LVN clinical experience in skilled nursing or similar setting
Compliance:
- Must meet facility background check, drug screen, and immunization requirements prior to start date
