Contract RN Case Manager opportunity in York, ME — 13-week assignment beginning July 6, 2026. Rotating shifts; $2,280/40 hours.

Overview
Seeking an experienced Registered Nurse to perform case management and utilization review for an inpatient hospital population. The RN will assess patient needs, coordinate transitions of care, lead interdisciplinary care planning, and support utilization and discharge processes to optimize outcomes and resource use.

Qualifications

• Associate’s or Bachelor’s degree in Nursing required; current RN licensure.
• 3–5 years clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review (minimum 3 years).
• Demonstrated utilization review and discharge planning experience.
• Active BLS required. CCM or ACM certification preferred; certification by a nationally recognized case management or utilization review organization is a plus.

Key Responsibilities

• Conduct comprehensive assessments to identify patient needs and determine resources required for safe transition to the next setting.
• Secure necessary resources by coordinating with clinical teams, payors, and community agencies.
• Lead and facilitate interdisciplinary care rounds, care conferences, and care planning sessions.
• Communicate effectively across written, verbal, and electronic systems, including electronic health records and email.
• Work with community organizations to arrange durable medical equipment, insurance authorizations, and patient transportation.
• Monitor and manage length of stay for inpatient populations, applying utilization review principles.
• Apply knowledge of hospital reimbursement methodologies and quality measures relevant to care management.
