Overview
Join Senior Care Partners PACE as a Registered Nurse delivering clinic- and home-based care for frail, elderly, and long-term care participants. This role requires independent clinical judgment, strong communication with PCPs, and regular home visits.

Qualifications

• Active RN diploma or degree from an accredited nursing program; associate required, bachelor’s in nursing preferred.
• Minimum 1 year nursing experience caring for frail, elderly, or long-term care populations.
• Home care and wound care experience preferred (at least 1 year each).
• Current AHA CPR certification or willingness to obtain.
• Reliable transportation, valid Michigan driver’s license, and auto insurance.
• Must be 18 years or older.
• Fully vaccinated—or willing to be fully vaccinated—against Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.
• Demonstrated commitment to compassion, integrity, teamwork, and excellence.

Key Responsibilities

• Triage emergent situations and independently apply advanced nursing knowledge to manage patient needs.
• Monitor and coordinate care for acute and chronic medical conditions with PCPs.
• Implement individualized care plans to deliver direct clinic and home-based nursing care.
• Administer prescribed treatments, procedures, and medications.
• Conduct regular home visits to perform wound care, lab draws, nursing assessments, and other ordered interventions.

Compensation & Schedule

• Contract/Travel assignment — 13 weeks, day shift.
• Rate: $1,860 per 40-hour week.
