Requirements
- Board Certified or truly Board Eligible in Plastic Surgery (must obtain certification within 5 years) — no exceptions
- Valid Iowa medical license
- Current, unrestricted federal and state authority to prescribe and administer medications as required by practice
- Active DEA registration
- BLS certification with ongoing recertification
- Clean malpractice and license history preferred
- Provide a self-query NPDB report dated within 30 days of submission
- Candidate visa status: H-1B cap-exempt (note in submission if applicable)
