Join a hospice/palliative care team in Missoula, MT for a 13-week contract where you'll provide compassionate, skilled nursing to patients and families in their homes. This role is a great fit for a confident RN with hospice, home health, or acute care experience who enjoys working collaboratively and managing clinical care from assessment through teaching and follow-up.

What you’ll do

  • Deliver skilled nursing and coordinate patient-centered care in a residential setting, following physician orders and working closely with the interdisciplinary team.
  • Assess and monitor patients’ physical and psychosocial status, identify changes or reactions to treatments, and plan and implement appropriate interventions.
  • Supervise and evaluate care provided by Home Health/Hospice Aides and CNAs to ensure adherence to the plan of care.
  • Communicate effectively with team members, community agencies, and families to ensure continuity of care; participate in case conferences as needed.
  • Provide symptom management, education, counseling, and referrals to support optimal comfort and quality of life.
  • Maintain accurate, current patient records in line with agency standards and OASIS requirements; stay informed about reimbursement rules and regulations.
  • Participate in scheduling and on-call rotations, triage patient needs by phone, and perform home visits when required.
  • Join quality improvement initiatives and attend required in-service training.
  • Perform other duties as assigned by your supervisor.

Requirements

  • Current RN license in Montana (BSN preferred).
  • Minimum 2 years of recent experience in acute care, home care, or hospice; clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred.
  • Valid driver’s license, appropriate auto insurance, and reliable transportation.
  • Strong communication skills, sound clinical judgment, and the ability to work independently and as part of a team.
