Shift: Days, Monday–Friday, 8-hour shifts (35 hrs/week). Start time must be consistent, communicated with Director of Rehab (DOR), and scheduled between 6:00 AM and 10:00 AM.
Weekends/On-call: None
Rate: $2,072 per 35-hour week

Responsibilities:

• Provide occupational therapy services to residents in long-term care/rehab setting.
• Evaluate, plan, implement, and document individualized treatment plans.
• Coordinate with interdisciplinary team and communicate progress with DOR.
• Maintain timely and accurate clinical documentation and billing compliance.
• Participate in care conferences and discharge planning as required.

Requirements:

• Minimum 1 year OT experience (LTC or rehab experience preferred)
• Active OT license in applicable state

Equipment/Resources: Facility provides standard OT tools and documentation systems (confirm specifics during orientation).
