Family Medicine Physician — Value-Based Advanced Primary Care (Phoenix, AZ)

We’re looking for experienced Family Medicine or Med-Peds physicians to join our growing Value-Based Advanced Primary Care team in Phoenix, AZ. This is a 100% on-site role delivering comprehensive primary care to a diverse patient population within an integrated, team-based setting. Ideal candidates have a minimum of five years of clinical experience and a genuine passion for patient-centered care. We also have openings in Gilbert, Peoria, and Scottsdale.

What we’re looking for

• MD or DO from an accredited medical school
• Board Certified or Board Eligible in Family Medicine or Med-Peds
• Preferably at least 5 years of clinical experience
• Current, unrestricted Arizona medical license
• Active DEA license and Arizona fingerprint clearance card
• BLS certification
• Strong verbal and written communication skills
• Excellent interpersonal skills and a collaborative mindset
• Comfortable working in value-based and integrated care models
• Basic computer skills and familiarity with EMR systems
• A commitment to outstanding patient service and professionalism
