Adult-Gerontology Nurse Practitioner (NP) — Mesa, AZ

Full-time position in Mesa, AZ providing comprehensive primary care for adults and older adults. The NP will perform assessments, diagnose and manage acute and chronic conditions, develop treatment plans, prescribe medications, and coordinate care with the interdisciplinary team to optimize patient outcomes.

Responsibilities

• Perform comprehensive history and physical exams for adult and geriatric patients.
• Diagnose and manage acute and chronic medical conditions.
• Develop and implement evidence-based treatment plans; prescribe medications within scope of practice.
• Provide preventive care, health maintenance, and chronic disease management.
• Conduct geriatric-specific assessments (functional status, cognition, polypharmacy, fall risk).
• Educate patients and caregivers on diagnoses, treatment plans, medications, and advance care planning.
• Coordinate care and communicate with physicians, specialists, therapists, and community resources.
• Maintain accurate, timely clinical documentation and comply with regulatory requirements.
• Participate in community outreach and patient advocacy as needed.

Qualifications

• Master's or Doctoral degree in Nursing with Adult-Gerontology NP focus.
• Current Arizona NP license.
• Certification as an Adult-Gerontology Nurse Practitioner.
• Clinical experience in adult and geriatric care.
• Strong clinical assessment, diagnostic, prescribing, and communication skills.
