Locums-to-Perm Nurse Practitioner or Physician Assistant — Hybrid Geriatric Primary Care (Bronx, NY)

Join a value-based primary care practice in The Bronx providing integrated outpatient and home-based services to geriatric patients. This locum-to-perm opportunity (conversion expected in 3–6 months) offers a hybrid schedule and guaranteed full-time hours.

Compensation: $75–$95 per hour

Schedule and Volume

• Full-time; Monday–Friday (5 days/week)
• Guaranteed hours
• Clinic: small daily census (7–10 patients)
• Home visits: 3–5 patients per day
• Low daily patient load to support comprehensive, patient-centered care

Key Responsibilities

• Provide comprehensive primary care for older adults, including preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe, and develop individualized care plans
• Order and interpret diagnostic tests (labs, imaging)
• Document accurately in the EHR and maintain regulatory compliance
• Educate patients and caregivers; promote wellness and care coordination
• Collaborate with physicians, nurses, and support staff; manage referrals and follow-up care

Qualifications

• Active NP or PA license in New York
• National board certification (NP or PA)
• Minimum 2 years’ primary care experience
• Medicaid and Medicare provider IDs required
• Spanish language proficiency preferred
• Expedited credentialing available
