Family or Internal Medicine Physician — Outpatient Primary Care in Colebrook, NH

We are seeking a dedicated, compassionate Board-Certified or Board-Eligible Family or Internal Medicine physician to join our Colebrook, NH team. This full-time, outpatient role provides comprehensive primary care to patients of all ages, with an emphasis on chronic disease management and care for an aging population. Physicians maintain individual patient panels and work within a collaborative clinical team that includes an RN Team Leader and a Medical Assistant.

Schedule & practice details

• Outpatient only, full-time (36 patient-contact hours + 4 administrative hours)
• Typical panel: 16–22 patient visits per day
• EMR: Meditech Expanse with Dragon dictation
• May participate in daytime call rotation; no after-hours call
• Clinical support from RN Team Leader and MA

Requirements

• MD or DO with Board Certification or Board Eligibility
• Completion of an accredited residency (ACGME, AOA, or RCPSC)
• Current ACLS and BLS certifications (AHA)
• Comfortable managing an independent patient panel
• Strong experience in chronic disease management, particularly with older adults
