Job Description
Locums-to-perm opportunity at a value-based primary care practice for geriatric patients. Hybrid role combining outpatient clinic and home-based visits.
Compensation
- $75–$85 per hour
Schedule & Patient Volume
- Monday–Friday, 5 days/week
- Home visits: 3–5 patients/day
- Clinic visits: 7–10 patients/day
Setting
- Hybrid: outpatient clinic and home-based care
- Value-based care model focused on elderly patients
Key Responsibilities
- Provide comprehensive primary care: preventive care, chronic disease management, acute care
- Diagnose, treat, prescribe, and create individualized treatment plans
- Order and interpret diagnostic tests (labs, imaging)
- Document thoroughly in the EHR
- Provide patient education and wellness counseling
- Coordinate care with physicians, nurses, support staff; manage referrals and follow-ups
- Ensure compliance with applicable state, federal, and professional regulations
Minimum Qualifications
- Active NY NP or PA license
- National board certification (NP or PA)
- Minimum 2 years’ primary care experience
- Spanish-speaking required
- Valid Medicaid and Medicare provider IDs required
