Requirements:

Key Responsibilities:

• Assess patients’ physical, cognitive, and environmental factors to identify functional limitations and establish measurable goals.
• Develop and implement individualized treatment plans using evidence-based therapeutic techniques to enhance daily living skills.
• Recommend and instruct patients in the use of adaptive equipment and assistive devices.
• Educate patients and families on strategies to maximize independence and manage conditions.
• Monitor and document patient progress, adjusting plans as needed, and coordinate care with the multidisciplinary team.
