Required Qualifications:

• Current, active Occupational Therapy state license.
• CPR and BLS certification from American Heart Association (AHA) or Red Cross (including AED).

Key Responsibilities:

• Assess patients’ physical and cognitive abilities and home/environmental factors to identify limitations and establish goals.
• Develop and implement individualized treatment plans using appropriate therapeutic techniques to improve functional independence and daily living skills.
• Recommend, train on, and monitor use of adaptive equipment and assistive devices.
• Educate patients and families on strategies for condition management and maximizing independence.
• Document assessments, treatment plans, interventions, and progress; adjust plans as needed.
• Communicate and coordinate care with the multidisciplinary healthcare team.
