Float Family Medicine Nurse Practitioner or Physician Assistant (Laurens, SC)

We are seeking a motivated Nurse Practitioner or Physician Assistant to join our Family Medicine team as a floating primary care provider. In this role you will deliver patient-centered care across multiple outpatient sites in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, focusing on prevention, chronic disease management, and continuity of care for patients across the lifespan.

Core responsibilities:

• Provide comprehensive, evidence-based primary care for children, adolescents, adults, and older adults.
• Travel regularly between assigned outpatient family medicine clinics across Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.
• Emphasize wellness, preventive care, and management of chronic conditions.
• Coordinate care with physicians, nurses, and the broader care team to ensure seamless patient management.
• Educate patients and families on diagnoses, treatment plans, medications, and health maintenance.
• Accurately and promptly document patient encounters in the electronic health record.

Qualifications:

• Graduate of an accredited NP or PA program (FNP preferred for NPs).
• Minimum two years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina license (or eligible to obtain).
• Board-certified or eligible in relevant specialty (FNP, PA-C).
