Adult-Gerontology Nurse Practitioner (NP) — Mesa, AZ

Full-time position in Mesa, AZ providing primary and specialty care for adult and older adult patients. The NP will perform comprehensive assessments, make diagnoses, develop and implement treatment plans, prescribe medications within scope, and coordinate care with multidisciplinary teams to achieve optimal patient outcomes.

Minimum Requirements

• Master’s or Doctoral degree in Nursing with Adult-Gerontology NP focus
• Current Arizona NP license
• Certification as an Adult-Gerontology Nurse Practitioner
• Clinical experience in adult and geriatric care

Core Responsibilities

• Conduct comprehensive patient assessments and establish diagnoses
• Provide primary care and manage acute and chronic conditions in adults and older adults
• Develop and implement individualized treatment plans; prescribe and adjust medications per scope of practice
• Coordinate care with physicians, specialists, nurses, and other healthcare professionals
• Deliver patient and family education on conditions, treatments, medications, and preventive care
• Apply geriatric-specific principles (functional assessment, polypharmacy review, cognitive and fall risk screening)
• Maintain timely, accurate clinical documentation and billing records
• Participate in community outreach and patient advocacy as needed
