Family or Internal Medicine Physician — Outpatient Primary Care (Colebrook, NH)

We’re looking for a compassionate, board-certified or board-eligible Family or Internal Medicine physician to join our Colebrook, NH team. This full-time, outpatient role focuses on high-quality, comprehensive primary care for patients of all ages, with an emphasis on chronic disease management and caring for an aging population. You’ll manage your own patient panel and work with a collaborative clinical team that includes an RN Team Leader and a Medical Assistant.

Schedule & practice details:

• Outpatient only, full-time schedule (36 patient-contact hours + 4 administrative hours)
• Typical patient volume: 16–22 patients per day
• EMR: Meditech Expanse with Dragon dictation
• May participate in a daytime call rotation (no after-hours call)
• Supported clinically by an RN Team Leader and Medical Assistant

Requirements:

• MD or DO, board-certified or board-eligible
• Completion of an accredited residency (ACGME, AOA, or RCPSC)
• Current ACLS and BLS certification through the American Heart Association
• Comfortable managing an independent patient panel
• Strong experience in chronic disease management, especially with elderly patients
