Mobile Adult/Geriatric Nurse Practitioner — Field-Based, Autonomy + Bonus in LA/Orange Counties

We are seeking a compassionate, field-based Adult/Geriatric Nurse Practitioner to provide acute, post-acute, and transitional care to medically complex and recently discharged patients across Los Angeles and Orange Counties.

In this role you will have clinical autonomy while collaborating with an interdisciplinary team to improve outcomes, reduce hospital readmissions, and support safe recovery in the home setting. Daily travel throughout assigned territories is required.

• Base salary: $119,313 – $178,968 annually

Qualifications

• Master of Science in Nursing
• Adult/Geriatric Nurse Practitioner certification
• Active, unrestricted California RN and NP licenses
• Minimum one year of NP clinical experience
• Strong assessment, diagnostic, and care-management skills
• Valid driver’s license and reliable transportation
• Ability to work independently and collaborate within a multidisciplinary team
• Current TB test or willingness to obtain prior to employment
