Required licensure & qualifications:

  • Current RN license in Montana (BSN preferred)
  • Valid driver’s license, appropriate auto insurance, and reliable transportation
  • Minimum 2 years recent experience in acute care, home care, or hospice (preferred)
  • Clinical skills: venipuncture, catheterization, wound care and/or parenteral therapy preferred

Core responsibilities:

  • Provide skilled nursing and coordinate physician-authorized care for patients and families in the home setting
  • Assess, observe, evaluate and document patients’ physical and psychosocial status; evaluate changes and treatment/medication responses
  • Develop, implement and revise individualized plans of care; maintain accurate patient records per agency standards and OASIS requirements
  • Supervise and evaluate Home Health/Hospice Aides and CNAs for assigned patients; ensure compliance with plan of care
  • Ensure continuity of care through effective communication, information exchange and case conferences with interdisciplinary team and community agencies
  • Provide symptom management, patient/family teaching and referrals to optimize clinical outcomes and quality of life
  • Participate in team scheduling, on-call rotation, telephone triage and home visits as needed
  • Participate in quality improvement activities and required in-service training
  • Perform other duties as assigned by supervisor
