Duties & Responsibilities
- Provide comprehensive professional nursing services across the continuum of hospice/palliative care, including patient admission, assessment, and development of individualized care plans
- Deliver direct patient care to clients of all ages
- Collaborate with interdisciplinary homecare team members and paraprofessionals to coordinate and maintain high-quality care
- Document care, communicate changes in patient condition, and participate in care planning and case reviews as needed
