- 2 days per week -- Monday & Tuesday preferred, 9 am - 5 pm
- 15 - 20 patients per day, increasing to 25 as clinic grows
- Ages 2+ patient population
- Active CAQH enrollment required
- Medicare/Medicaid enrollment required -- PTAN number needed
- Hospital privileges required
- Paid malpractice insurance; pre-paid travel and housing expenses
- Assignment details and time entry in online portal
