Job Description
Join a team caring for residents in skilled nursing facilities around New Hope, Minnesota. This part-time role lets you provide hands-on, patient-centered care across local post-acute and long-term care settings while working alongside experienced Nurse Practitioners and Physician Assistants. You’ll provide clinical leadership and oversight for assigned facilities, with a flexible schedule that supports work-life balance and meaningful impact.
We also have additional facility opportunities nationwide in Georgia, Illinois, Indiana, Kansas, Louisiana, Massachusetts, Michigan, Missouri, Ohio, Tennessee, and Wisconsin.
What this role looks like
- Part-time coverage of local skilled nursing facilities, typically 1–2 days per week at each site
- No fixed daily start/end times, no call, and no overnight shifts
- Expect about 18–25 visits per day
- Collaborative practice with NPs and PAs and a supportive team culture
- Company-provided iPad with LTE for efficient documentation
- New graduates who are board-eligible and working toward licensure are welcome to apply

Basic qualifications
- MD or DO with an active, unrestricted U.S. medical license or eligibility for licensure
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
- Strong communication, leadership, and clinical decision-making skills
- A commitment to compassionate, patient-centered care
