Job Description
Join a growing Post-Acute Care team in Burlington, VT as a full-time Nurse Practitioner or Physician Assistant providing patient-centered care for individuals transitioning from acute hospitals to skilled nursing and rehabilitation facilities.
Key responsibilities
- Provide facility-based rounding and management of acute and chronic medical conditions in post-acute care settings.
- Work collaboratively with interdisciplinary teams to support recovery, coordinate care, and minimize hospital readmissions.
- Perform clinical assessments, diagnostics, and treatment planning; adjust care plans based on patient progress.
- Communicate effectively with patients, families, nursing staff, and other providers; document care in the EMR.
- Practice with autonomy while leveraging available clinical and operational support.
Position details
- Schedule: Monday–Friday, 9:00 AM–5:00 PM
- No weekends, no on-call responsibilities
- Facility-based rounding in skilled nursing and rehabilitation centers
- Opportunity to build long-term relationships with patients and care teams
Compensation and benefits
- Base salary: $120,000 annually
- Total earning potential: $180,000–$220,000+

Qualifications
- Graduate of an accredited NP or PA program
- Current, unrestricted NP or PA license in the applicable state, or eligibility for licensure
- National board certification (NP or PA)
- DEA registration or eligibility to obtain
- Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal abilities
- Ability to work independently and as part of a multidisciplinary team
- Proficiency with EMR systems
