Hospitalist NP/PA — Skilled Nursing Focus in Cleveland, OH | Flexible 6Hour Shifts, No Nights/Call

Location: Cleveland, Ohio — skilled nursing and post-acute facilities

Role overview
Provide patient-centered primary and acute care to older adults in skilled nursing facilities. Work in a collaborative, team-based setting focused on managing chronic conditions, acute issues, and transitions of care.

Schedule and clinical duties

• Full-time with flexible scheduling; typical facility shift: six hours, rounds begin between 7:00 a.m. and 10:00 a.m.
• No on-call or overnight responsibilities
• Perform daily rounds, assess and manage acute and chronic conditions, initiate and adjust medications and treatment plans, and coordinate care with facility staff and families
• Document efficiently using streamlined EHR with AI Scribe and voice dictation; company-provided iPad with LTE
• Access UpToDate® for clinical decision support

Qualifications

• Active, unrestricted Nurse Practitioner license (Ohio)
• National certification (e.g., ANCC, AANPCB, AACN) required
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
