Post-Acute Care Nurse Practitioner or Physician Assistant — Full-Time, North Conway (No Weekends, No Call)

Location: North Conway, NH (additional openings in Wakefield, RI and North Conway, NH)

Role overview:
Join a post-acute care team providing medical management for patients transitioning from hospital to skilled nursing and rehabilitation facilities. Deliver patient-centered care to manage acute and chronic conditions, support recovery, reduce readmissions, and optimize outcomes in facility-based settings.

Key responsibilities:

• Perform facility-based rounding in post-acute care settings
• Evaluate, diagnose, and manage acute and chronic medical conditions
• Coordinate care with interdisciplinary teams, families, and outside providers
• Develop and implement treatment plans and transitions-of-care strategies
• Monitor clinical progress and adjust care plans as needed
• Document encounters and maintain accurate medical records in the EMR
• Provide education to patients, families, and facility staff
• Support quality improvement and readmission reduction efforts

Schedule and work environment:

• Full-time, Monday–Friday, 9:00 AM–5:00 PM
• No weekends, no on-call responsibilities
• Autonomy in clinical practice with clinical and operational support

Compensation and benefits:

• Base salary: $120,000 annually
• Total earning potential: $180,000–$220,000+

Qualifications:

• Graduate of an accredited NP or PA program
• Current, unrestricted NP or PA license in applicable state or eligibility for licensure
• National board certification (NP or PA)
• DEA registration or eligibility to obtain
• Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
• Strong clinical assessment, diagnostic, and care coordination skills
• Excellent communication and interpersonal skills
• Ability to work independently and collaborate within a multidisciplinary team
• Proficiency with EMR systems
