Location: North Conway, NH (additional openings in Wakefield, RI and North Conway, NH)
Role overview:
Join a post-acute care team providing medical management for patients transitioning from hospital to skilled nursing and rehabilitation facilities. Deliver patient-centered care to manage acute and chronic conditions, support recovery, reduce readmissions, and optimize outcomes in facility-based settings.
Key responsibilities:
- Perform facility-based rounding in post-acute care settings
- Evaluate, diagnose, and manage acute and chronic medical conditions
- Coordinate care with interdisciplinary teams, families, and outside providers
- Develop and implement treatment plans and transitions-of-care strategies
- Monitor clinical progress and adjust care plans as needed
- Document encounters and maintain accurate medical records in the EMR
- Provide education to patients, families, and facility staff
- Support quality improvement and readmission reduction efforts
Schedule and work environment:
- Full-time, Monday–Friday, 9:00 AM–5:00 PM
- No weekends, no on-call responsibilities
- Autonomy in clinical practice with clinical and operational support
- Opportunity to build long-term relationships with patients and care teams
Compensation and benefits:
- Base salary: $120,000 annually
- Total earning potential: $180,000–$220,000+

Qualifications:
- Graduate of an accredited NP or PA program
- Current, unrestricted NP or PA license in applicable state or eligibility for licensure
- National board certification (NP or PA)
- DEA registration or eligibility to obtain
- Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal skills
- Ability to work independently and collaborate within a multidisciplinary team
- Proficiency with EMR systems
