Post-Acute Care Nurse Practitioner or Physician Assistant – Wakefield & North Conway

Join a growing Post-Acute Care team in Wakefield, RI (additional openings in Wakefield, RI and North Conway, NH) as a Nurse Practitioner or Physician Assistant providing patient-centered care to individuals transitioning from acute hospital settings to skilled nursing and rehabilitation facilities. This full-time role focuses on managing chronic and acute medical needs, supporting recovery, reducing readmissions, and optimizing outcomes through collaborative, interdisciplinary care.

Schedule and work environment

• Full-time, Monday–Friday, 9:00 AM–5:00 PM
• No weekends or on-call responsibilities
• Facility-based rounding in post-acute care settings
• Clinically autonomous role with strong operational support

Compensation and benefits

• Base salary: $120,000 annually
• Total earning potential: $180,000–$220,000+

Key responsibilities

• Perform comprehensive assessments, diagnose, and manage acute and chronic conditions
• Coordinate care and communicate with interdisciplinary teams, families, and facility staff
• Implement plans to support recovery, prevent readmission, and promote functional progress
• Document care and manage clinical workflows within the EMR

Qualifications

• Graduate of an accredited NP or PA program
• Current, unrestricted NP or PA license in the applicable state, or eligibility for licensure
• National board certification (NP or PA)
• DEA registration or eligibility to obtain
• Minimum one year of clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
• Strong clinical assessment, diagnostic, and care coordination skills
• Excellent communication and interpersonal abilities
• Ability to work independently while collaborating effectively within a multidisciplinary team
• Proficiency with EMR systems
