Job Description
Join a growing Post-Acute Care team in Wakefield, RI (additional openings in Wakefield, RI and North Conway, NH) as a Nurse Practitioner or Physician Assistant providing patient-centered care to individuals transitioning from acute hospital settings to skilled nursing and rehabilitation facilities. This full-time role focuses on managing chronic and acute medical needs, supporting recovery, reducing readmissions, and optimizing outcomes through collaborative, interdisciplinary care.
Schedule and work environment
- Full-time, Monday–Friday, 9:00 AM–5:00 PM
- No weekends or on-call responsibilities
- Facility-based rounding in post-acute care settings
- Clinically autonomous role with strong operational support
- Opportunity to build long-term relationships with patients and care teams
Compensation and benefits
- Base salary: $120,000 annually
- Total earning potential: $180,000–$220,000+

Key responsibilities
- Perform comprehensive assessments, diagnose, and manage acute and chronic conditions
- Coordinate care and communicate with interdisciplinary teams, families, and facility staff
- Implement plans to support recovery, prevent readmission, and promote functional progress
- Document care and manage clinical workflows within the EMR
Qualifications
- Graduate of an accredited NP or PA program
- Current, unrestricted NP or PA license in the applicable state, or eligibility for licensure
- National board certification (NP or PA)
- DEA registration or eligibility to obtain
- Minimum one year of clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal abilities
- Ability to work independently while collaborating effectively within a multidisciplinary team
- Proficiency with EMR systems
