Job Description

Join a growing team delivering patient-centered primary care to residents of skilled nursing facilities across Bainbridge, Georgia. This part-time role offers flexible scheduling with no on-call or overnight duties, collaborative work with physicians and facility staff, and the chance to provide evidence-based care in a fast-paced SNF setting.

Schedule and Clinical Duties

  • Part-time with flexible hours; six-hour facility shifts
  • Rounds typically start between 7:00 a.m. and 10:00 a.m.
  • Average 15–20 patient visits per day
  • Conduct daily rounds, assess and manage acute and chronic conditions, prescribe medications, and develop individualized treatment plans
  • Collaborate with nursing staff and interdisciplinary teams to coordinate care
  • Maintain clinical and regulatory compliance
  • Company-provided iPad with LTE

Qualifications

  • Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
  • NP certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
  • Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
  • Strong clinical judgment, communication skills, and patient-centered approach
  • Ability to work independently while collaborating effectively with a multidisciplinary team
