NP/PA opportunity for Senior focused Primary Care clinic in Great Falls, MT

Great opportunity for a Nurse Practitioner or Physician Assistant to live and work in beautiful Great Falls, MT for a fast growing organization delivering care to older adults in access starved communities. 

• Board Certified Physician Assistant (PA) or Nurse Practitioner (NP) - FNP, ANP, ACNP or AGNP
• Must have 3+ yrs exp in Primary Care and independent patient care
• Will be focused on outcomes and not volume - prioritize quality over quantity
• Manage a patient panel, supported by the MA/LPN's, Front Office Coordinator, Clinic Manager, Medical Director, and additional teammates 
• Full scope of primary care services including well acute, and chronic conditions
• Perform Annual Wellness Visits and Health Risk Assessments
• Medicare Advantage
• Must be skilled with HCC coding
