Job Description
Join a supportive team providing compassionate care to post-acute and long-term care patients across skilled nursing facilities in Toledo, Ohio. This part-time role gives you flexible scheduling and the chance to lead clinically while collaborating closely with experienced Nurse Practitioners and Physician Assistants. You’ll provide hands-on, patient-centered care and clinical oversight at assigned facilities, helping improve outcomes for residents in a community-focused setting.
What the role looks like
- Part-time coverage of local skilled nursing facilities (typically 1–2 days per week per facility)
- No fixed start/end times, no overnight shifts, and no on-call responsibilities
- Average caseload of 18–25 visits per day
- Work alongside NPs and PAs while providing clinical leadership and decision-making
- Flexible schedule that supports a strong work–life balance
- Company-provided iPad with LTE for streamlined documentation

Who we’re looking for
- MD or DO with an active, unrestricted U.S. medical license or eligibility for licensure
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, Physical Medicine & Rehabilitation, or Emergency Medicine
- Strong communicator with leadership ability and sound clinical judgment
- A commitment to providing compassionate, patient-focused care
- New graduates who are board-eligible and actively pursuing licensure are encouraged to apply
