Job Description
Join a growing Post-Acute Care team in Springfield, MA where you’ll provide compassionate, patient-centered care to individuals transitioning from the hospital to skilled nursing and rehabilitation facilities. This full-time role offers a chance to make a real difference by supporting recovery, managing chronic and acute conditions, and helping reduce hospital readmissions while building lasting relationships with patients and care teams.
What you’ll do:
- Round in facility-based post-acute care settings, working closely with interdisciplinary teams
- Assess and manage chronic and acute medical issues, create treatment plans, and coordinate follow-up care
- Support rehabilitation goals and help optimize patient outcomes during transitions of care
- Practice with a high degree of clinical autonomy while benefiting from strong operational and clinical support
- Communicate clearly with patients, families, and care teams to ensure seamless care coordination
Schedule and work environment:
- Full-time, Monday–Friday, 9:00 AM–5:00 PM
- No weekends and no on-call responsibilities
- Facility-based rounding in post-acute/long-term care settings
- Opportunity to develop long-term relationships with patients and interdisciplinary teams
Compensation and benefits:
- Base salary: $120,000 annually
- Total earning potential: $180,000–$220,000+

What we’re looking for:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current, unrestricted NP or PA license (or eligibility for licensure) in the state
- National board certification as an NP or PA
- DEA registration or eligibility to obtain
- Minimum of one year of clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
- Strong clinical assessment, diagnostic, and care coordination skills
- Excellent communication and interpersonal abilities
- Comfortable working independently while collaborating effectively within a multidisciplinary team
- Proficiency with EMR systems
