Job Description

Join a growing Post-Acute Care team in Burlington, VT, delivering high-quality, patient-centered care to individuals transitioning from hospital to skilled nursing and rehabilitation settings. This full-time role focuses on managing chronic and acute medical conditions, supporting recovery, reducing readmissions, and optimizing outcomes through close collaboration with interdisciplinary teams.

Key responsibilities

  • Perform facility-based rounding in post-acute care settings, providing comprehensive assessments, diagnosis, treatment, and care planning.
  • Coordinate with nursing staff, therapists, social workers, and other clinicians to ensure continuity of care and effective discharge planning.
  • Manage medication regimens, acute episodes, and chronic disease management to promote recovery and prevent rehospitalization.
  • Document care in the EMR and participate in quality improvement initiatives.
  • Build longitudinal relationships with patients, families, and facility teams.

Schedule and work environment

  • Full-time, Monday–Friday, 9:00 AM–5:00 PM
  • No weekends, no on-call responsibilities
  • Autonomous clinical practice with strong operational and multidisciplinary support

Compensation and benefits

  • Base salary: $120,000 annually
  • Total earning potential: $180,000–$220,000+ (based on productivity/earnings model)

Qualifications

  • Graduate of an accredited NP or PA program
  • Current, unrestricted NP or PA license in the applicable state or eligibility for licensure
  • National board certification and DEA registration or eligibility to obtain
  • Minimum one year clinical experience preferred; experience in post-acute care, skilled nursing, geriatrics, internal medicine, family medicine, hospital medicine, or long-term care strongly preferred
  • Strong clinical assessment, diagnostic, and care coordination skills
  • Excellent communication and interpersonal abilities; ability to work independently and as part of a multidisciplinary team
  • Proficiency with electronic medical records
