Hospitalist Nurse Practitioner / Physician Assistant — Skilled Nursing (Toledo, OH)

We seek compassionate Nurse Practitioners (and Physician Assistants) dedicated to improving the health and quality of life of older adults in a collaborative, team-based setting.

Locations: Toledo, OH, with additional opportunities nationwide (FL, GA, IL, MA, MI, NH, NC, WI).

What you’ll do

• Provide daily rounds and longitudinal care in skilled nursing facilities, managing acute and chronic conditions and prescribing treatments.
• Work six-hour facility shifts (rounds typically start between 7:00 a.m. and 10:00 a.m.).
• No overnight or on-call duties.
• Document efficiently using a streamlined EHR supported by AI scribe and voice dictation on a company-provided iPad with LTE.
• Access UpToDate® for point-of-care clinical guidance.

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant license.
• Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred.
