Locums-to-Perm Nurse Practitioner or Physician Assistant — Hybrid Geriatric Primary Care (Queens, NY)

Locums-to-perm opportunity (convert within 3–6 months) for an experienced Nurse Practitioner or Physician Assistant in a value-based primary care practice serving geriatric patients. Hybrid role with both outpatient clinic and home-based patient visits.

Compensation

• $75–$95 per hour

Schedule & Workload

• Full-time, guaranteed hours
• Monday–Friday, 5 days/week; 8:30 a.m. start
• Low daily census
  • Clinic: ~7–10 patients/day
  • Home visits: ~3–5 patients/day

Setting

• Hybrid: outpatient clinic + home-based care
• Value-based primary care focused on elderly patients

Primary Responsibilities

• Provide comprehensive primary care for geriatric patients: preventive care, chronic disease management, and acute care
• Diagnose, treat, prescribe, and develop individualized care plans
• Order and interpret diagnostic tests (labs, imaging)
• Document care accurately in the EHR
• Provide patient education and wellness counseling
• Coordinate care with physicians, nurses, and support staff; manage referrals and follow-ups
• Ensure compliance with applicable state, federal, and professional regulations

Qualifications

• Active NY NP or PA license
• Minimum 2 years’ primary care experience (geriatrics experience preferred)
• National board certification (NP/PA)
• Medicaid and Medicare provider IDs required
• Ability to complete expedited credentialing process
