Locums-to-Perm NP/PA — Hybrid Geriatric Primary Care, Low Panel & Home Visits (NYC)

We are seeking an experienced Nurse Practitioner or Physician Assistant for a locum-to-perm assignment (conversion expected within 3–6 months) at a value-based primary care practice serving geriatric patients in New York City. This full-time, hybrid role combines outpatient clinic shifts with home-based visits and emphasizes coordinated, preventive, and chronic care for older adults.

Compensation: $75–$95/hour

Schedule & Patient Volume:

• Full time, guaranteed hours
• Monday–Friday, 5 days/week, typical start at 8:30 AM
• Clinic: small daily census (approximately 7–10 patients)
• Home visits: 3–5 patients per day
• Low daily patient load to allow focused, comprehensive care

Key Responsibilities:

• Provide comprehensive primary care for geriatric patients, including preventive care, chronic disease management, and acute problem assessment and treatment
• Diagnose, prescribe, and develop individualized treatment plans
• Order and interpret diagnostic tests (labs, imaging) and manage follow-up care and specialist referrals
• Document thoroughly and accurately in the EHR
• Deliver patient education and wellness counseling tailored to older adults
• Collaborate with physicians, nurses, and care team members to ensure coordinated, high-quality care
• Maintain compliance with applicable state, federal, and professional regulations

Qualifications:

• Active NP or PA license in New York State
• Minimum 2 years of primary care experience (geriatric experience preferred)
• National board certification
• Ability to obtain expedited credentialing
• Medicaid and Medicare provider IDs required or ability to enroll
