Mobile Adult/Geriatric Nurse Practitioner — Field-Based Home Care (Los Angeles, CA)

We’re looking for a Mobile Adult/Geriatric Nurse Practitioner to provide acute, post-acute, and transitional care to medically complex and recently discharged patients across Los Angeles County, CA.

This field-based role blends clinical autonomy with team-based support. You’ll work closely with an interdisciplinary care team to improve outcomes, reduce readmissions, and help patients recover safely and comfortably at home.

What you’ll do

• Visit patients in their homes to perform assessments, diagnose issues, develop and implement treatment plans, and coordinate follow-up care
• Manage medically complex and recently discharged adults and older adults
• Communicate and collaborate with physicians, therapists, social workers, and caregivers to ensure continuity of care
• Provide patient and family education to support recovery and prevent readmission
• Document encounters and care plans in the electronic medical record

• Base salary range: $119,313 – $178,968 annually

What we’re looking for

• Master of Science in Nursing
• Adult/Geriatric Nurse Practitioner certification
• Active, unrestricted California RN and NP licenses
• At least 1 year of clinical experience as a Nurse Practitioner
• Strong assessment, diagnostic, and care-management skills
• Valid driver’s license and reliable transportation
• Ability to work independently while engaging collaboratively with a multidisciplinary team
• Current TB test or willingness to obtain prior to employment
