Mobile Adult/Geriatric Nurse Practitioner — Field-Based, Autonomy, Bonus & Benefits (San Fernando)

We are hiring a Mobile Adult/Geriatric Nurse Practitioner to provide acute, post-acute, and transitional care to medically complex and recently discharged patients across the San Fernando Valley and Antelope Valley.

This field-based role offers clinical autonomy within an interdisciplinary team focused on improving patient outcomes, lowering readmissions, and supporting safe recovery in the home environment.

Key responsibilities and features

• Provide comprehensive in-home assessments, diagnosis, treatment planning, and care management for medically complex and recently discharged adult/geriatric patients
• Deliver acute, post-acute, and transitional care with a relationship-centered, value-based approach
• Collaborate with physicians, nurses, therapists, social workers, and other team members to coordinate care and optimize outcomes
• Travel within assigned territories; manage caseload independently while engaging the care team as needed

Compensation and benefits

• Base salary: $119,313 – $178,968 annually, plus performance-based bonus opportunity

Qualifications

• Master of Science in Nursing and Adult/Geriatric Nurse Practitioner certification
• Active, unrestricted California RN and NP licenses
• Minimum of 1 year of NP clinical experience
• Strong assessment, diagnostic, and care management skills
• Valid driver’s license and reliable transportation
• Ability to work independently and collaboratively within a multidisciplinary team
• Current TB test or willingness to obtain prior to employment
