Family or Internal Medicine Physician — Geriatric SNF/LTC Primary Care (Tucson, AZ)

We are seeking a Board Certified or Board Eligible Family Medicine or Internal Medicine physician to deliver comprehensive primary care for Institutional Special Needs Plan members across Tucson, Arizona.

You will practice value-based medicine with an emphasis on preventive care, chronic disease management, care coordination, and measurable quality outcomes for medically complex geriatric patients. Work collaboratively within a multidisciplinary team to provide compassionate, patient-centered care in post-acute, SNF, and long-term care environments.

Qualifications

• MD or DO from an accredited medical school
• Active Arizona medical license or eligibility to obtain one
• Current DEA registration
• Board Certified or Board Eligible in Family Medicine or Internal Medicine
• Preferred experience with geriatric, post-acute care, SNF/LTC, or medically complex populations
• Strong clinical judgment, communication, and teamwork skills
• Commitment to compassionate, patient-centered care
