Float Nurse Practitioner or Physician Assistant — Family Medicine (Laurens, SC & Regional)

We are hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to serve as a floating primary care provider across outpatient family medicine sites in Abbeville, Greenwood, Laurens, McCormick, and Saluda counties, SC.

Responsibilities

• Provide comprehensive primary care for patients across the lifespan: wellness, prevention, acute care, and chronic disease management.
• Travel regularly to assigned family medicine clinics within the five-county region; reliably cover scheduled clinic shifts.
• Perform patient assessments, develop and implement treatment plans, prescribe and manage medications per scope of practice.
• Coordinate care with physicians, nurses, and ancillary staff; facilitate referrals and follow-up.
• Educate patients and families on diagnoses, treatments, medications, and preventive health.
• Accurately document encounters in the electronic health record in a timely manner and complete required administrative tasks.

Requirements

• Graduate of an accredited NP (APRN) or PA program.
• Minimum 2 years’ experience in primary care or family medicine.
• Active, unrestricted South Carolina license (or eligible).
• National certification (FNP, PA-C) or eligibility for certification.
• Valid driver’s license and reliable transportation for inter-site travel.
• Strong clinical judgment, communication skills, and ability to work independently in varied clinic settings.
