Job Description
We’re expanding a compassionate, team-based practice that delivers high-quality, patient-centered care to residents in skilled nursing facilities across Ossipee, New Hampshire. If you love working with older adults and want to make a meaningful impact on their health and quality of life, you’ll fit right in. We’re looking for Nurse Practitioners and Physician Assistants who value collaboration, clinical autonomy, and efficient workflows.
What you can expect
- Full-time roles with flexible schedules and six-hour facility shifts (rounds typically start between 7:00 a.m. and 10:00 a.m.).
- No overnight shifts and no on-call duties.
- Daily rounds managing both acute and chronic conditions, developing treatment plans, and prescribing medications.
- Efficient documentation supported by a streamlined EHR, AI scribe, voice dictation, and a company-provided iPad with LTE.
- Access to UpToDate® for real-time clinical guidance.

Basic requirements
- Active, unrestricted Nurse Practitioner or Physician Assistant license.
- Board certification (ANCC, AANPCB, PNCB, AACN, or NCCPA).
- Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
