Adult-Gerontology Nurse Practitioner (NP) — Mesa, AZ

We’re looking for a full-time Adult-Gerontology Nurse Practitioner to join our team in Mesa, AZ. In this role you’ll provide compassionate, evidence-based care to adult and older adult patients—performing thorough assessments, making diagnoses, developing treatment plans, prescribing as appropriate, and working closely with colleagues to deliver coordinated care.

What you’ll do:

• Conduct comprehensive assessments and diagnose acute and chronic conditions in adults and older adults.
• Provide primary care services including preventive care, chronic disease management, and acute visit management.
• Develop individualized treatment plans and prescribe medications within scope of practice.
• Communicate and collaborate with physicians, specialists, nurses, and other care team members to ensure seamless patient care.
• Educate patients and their families about conditions, medications, lifestyle changes, and advance care planning.
• Apply geriatric-specific knowledge to address mobility, cognition, polypharmacy, falls risk, and functional status.
• Maintain accurate, timely documentation and follow charting standards.
• Participate in community outreach and patient advocacy to improve access and health outcomes for older adults.

What we’re looking for:

• Master’s or Doctoral degree in Nursing with a focus on Adult-Gerontology Nurse Practitioner.
• Current Arizona NP licensure.
• Certification as an Adult-Gerontology Nurse Practitioner.
• Clinical experience caring for adult and geriatric populations.
