Hospitalist Nurse Practitioner / Physician Assistant — Skilled Nursing & Long-Term Care (Agawam, MA)

We’re looking for experienced Nurse Practitioners who are committed to improving the health and quality of life of older adults in a collaborative, team-based setting.

Locations and flexibility

• Primary opening in Agawam, MA, with additional opportunities nationwide (IL, MI, NH, NC, OH, WI).
• Full-time roles with flexible scheduling to fit your life.

What your day looks like

• Six-hour facility shifts for full-time clinicians, with rounds typically starting between 7:00–10:00 a.m.
• No overnight shifts or on-call responsibilities.
• Perform daily rounds, manage acute and chronic conditions, develop and prescribe treatment plans, and coordinate care with facility teams.

• Streamlined EHR with AI scribe and voice dictation to minimize documentation burden.
• Company-provided iPad with LTE for easy access on the go.
• Access to UpToDate® for reliable, real-time clinical guidance.

Who we’re looking for

• Active, unrestricted Nurse Practitioner license and national board certification (ANCC, AANPCB, PNCB, or similar).
• Experience in skilled nursing, long-term care, geriatrics, or post-acute care is preferred.
• Compassionate, team-oriented clinicians who enjoy working with older adults.
