Job Description
Provide patient-centered primary care to residents in skilled nursing facilities across Mishawaka, IN. Part-time, flexible schedule with no call or overnight responsibilities. Work collaboratively with physicians, facility staff, and interdisciplinary teams to manage acute and chronic conditions and ensure regulatory compliance.
Locations
- Mishawaka, Indiana (additional opportunities nationwide: GA, IL, KS, MI, MO, OH, TN, WI)
Schedule & Workload
- Part-time; six-hour facility shifts
- Rounds typically start between 7:00 a.m. and 10:00 a.m.
- No call or overnight shifts
- Average 15–20 patient visits per day
Core Responsibilities
- Conduct daily rounds and bedside assessments
- Diagnose and manage acute and chronic conditions
- Prescribe and adjust medications; develop treatment plans
- Coordinate care with nursing staff and interdisciplinary teams
- Maintain clinical documentation and comply with regulatory standards

Qualifications
- Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
- NP board certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
- Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
- Strong clinical judgment, communication, and independent practice skills
- Ability to collaborate effectively within a multidisciplinary team
