Part-Time Hospitalist Nurse Practitioner / Physician Assistant — Skilled Nursing Facilities (Mishawaka, IN)

Provide patient-centered primary care to residents in skilled nursing facilities across Mishawaka, IN. Part-time, flexible schedule with no call or overnight responsibilities. Work collaboratively with physicians, facility staff, and interdisciplinary teams to manage acute and chronic conditions and ensure regulatory compliance.

Locations

• Mishawaka, Indiana (additional opportunities nationwide: GA, IL, KS, MI, MO, OH, TN, WI)

Schedule & Workload

• Part-time; six-hour facility shifts
• Rounds typically start between 7:00 a.m. and 10:00 a.m.
• No call or overnight shifts
• Average 15–20 patient visits per day

Core Responsibilities

• Conduct daily rounds and bedside assessments
• Diagnose and manage acute and chronic conditions
• Prescribe and adjust medications; develop treatment plans
• Coordinate care with nursing staff and interdisciplinary teams
• Maintain clinical documentation and comply with regulatory standards

Qualifications

• Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license
• NP board certification: ANCC, AANPCB, PNCB, or AACN; PA certification: NCCPA
• Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
• Strong clinical judgment, communication, and independent practice skills
• Ability to collaborate effectively within a multidisciplinary team
