Job Description

Location: Coulterville, IL (serving local skilled nursing facilities). Additional facility opportunities across Illinois: Carlinville, Casey, Flanagan, Galesburg, Hillsboro, Macomb, Marshall, Mascoutah, Paris, Springfield. Nationwide opportunities also available in select states.

Role summary:
Provide part-time clinical leadership and attendingship for post-acute and long-term care patients in skilled nursing facilities. Collaborate with Nurse Practitioners and Physician Assistants to deliver patient-centered care and oversee clinical operations within assigned facilities.

Key responsibilities:

  • Admit, evaluate, and manage patients in skilled nursing facilities.
  • Provide clinical oversight and guidance to NPs and PAs.
  • Perform 18–25 patient visits per day (typical).
  • Coordinate care plans, transitions, and follow-up.
  • Ensure documentation and compliance using company-provided iPad with LTE.
  • Participate in performance improvement and quality initiatives as needed.

Schedule and work pattern:

  • Part-time; physicians typically cover each facility 1–2 days per week.
  • Flexible schedule with no fixed start/end times.
  • No call, no overnight shifts.

Qualifications:

  • MD or DO with unrestricted U.S. medical license or eligible for licensure.
  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine.
  • Strong clinical decision-making, leadership, and communication skills.
  • Commitment to compassionate, patient-centered care.
