Medical Director / Attending Physician Skilled Nursing (Hospitalist) — Agawam, MA

We are hiring a full-time Medical Director/Attending Physician to provide clinical leadership and deliver care in skilled nursing facilities in East and North Agawam, MA. You will collaborate with nurse practitioners and physician assistants to manage post-acute and long-term care patients.

Responsibilities

• Provide onsite patient care and clinical oversight at assigned skilled nursing facilities
• Partner with NPs/PAs to coordinate treatment plans and transfers, manage acute changes, and perform follow-up visits
• Serve as clinical leader for the facility, guiding staff and participating in quality initiatives
• Document encounters using company-provided iPad with LTE

Schedule and Clinical Load

• Full-time; physicians typically cover each facility 1–2 days per week
• No fixed daily start/end times, no overnight shifts, and no on-call responsibilities
• Average 18–25 visits per day
• Flexible schedule to support work–life balance

Qualifications

• MD or DO with unrestricted U.S. medical license or eligibility for licensure
• Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine
• Strong clinical decision-making, leadership, and communication skills
• Commitment to compassionate, patient-centered care
