Responsibilities

• Provide outpatient family medicine care, including preventive, acute, and chronic disease management
• Perform clinical assessments, develop and implement treatment plans
• Maintain accurate, compliant medical records
• Coordinate care with interdisciplinary team members

Qualifications

• MD or DO from an accredited medical school
• Completion of an accredited U.S. Family Practice residency
• Board certified in Family Medicine
• Valid Arizona medical license and DEA certificate
• Current BLS certification
• Minimum three years’ experience in community health
• Bilingual English/Spanish preferred
• Competency working with diverse cultural and socioeconomic populations
• Proficiency with basic clinical methods and electronic medical records
• Basic computer skills and business English proficiency (spelling, punctuation)
• Valid Arizona driver’s license
